Please fill out this registration form for the 16th - 18th of July 2010 Waynesville Youth Football Camp.  If you have more than one child fill out one form per child.  When finished, please submit form.  Forms must be printed and signed and brought to camp the first day.

I hereby authorize the directors of the Waynesville Youth Football League camp to act for me according to their best judgment in any emergency requiring medical attention.  I know of no mental or physical problems which might affect my son/daughter's ability to safely participate in camp.  I will be responsible for any medical or any other charges in connection with his attendance at the camp.  I agree to abide by any rules and regulations of the camp.
